(adrenaline,epinephrine,AD)是上腺髓質(zhì)的主要激素,其 生物合成主要是在髓質(zhì)鉻細(xì)胞中首先形成去甲上腺素,然后進 一步經(jīng)苯乙胺-N-甲基轉(zhuǎn)移酶(phenylethanolamine N-methyl  transferase,PNMT)的作用,使去甲上腺素甲基化形成上腺 素,其化學(xué)本質(zhì)為兒茶酚胺。上腺素的一般作用使心臟收縮力上升;心臟、肝、和筋骨的血 管擴張和皮膚、粘膜的血管縮小。在藥物上,上腺素在心臟停 止時用來刺激心臟,或是哮喘時擴張氣管。上腺素能使心肌收縮力加強、興奮性增高,傳導(dǎo)加速,心輸出 量增多。對全身各部分血管的作用,不僅有作用強弱的不同,而 且還有收縮或舒張的不同。對皮膚、粘膜和內(nèi)臟(如臟)的血 管呈現(xiàn)收縮作用;對冠狀動脈和骨骼肌血管呈現(xiàn)擴張作用等。由 于它能直接作用于冠狀血管引起血管擴張,改善心臟供血,因此 是一種作用快而強的強心藥。上腺素還可松弛支氣管平滑肌及 解除支氣管平滑肌痙攣。利用其興奮心臟收縮血管及松弛支氣管 平滑肌等作用,可以緩解心跳微弱、血壓下降、呼吸困難等癥狀 。小腸內(nèi)有兩種腺體,即十二指腸腺和腸腺。十二指腸腺又稱勃 氏腺,是分布于十二指腸粘膜下層的一種分支管泡狀腺。十二指 腸腺分泌堿性液體,內(nèi)含粘蛋白,其粘稠度很高,主要是防止十 二指腸上皮免受胃酸侵蝕。腸腺又稱李氏腺,分布于全部小腸的 粘膜層,它是不分支的直管狀腺,其分泌物構(gòu)成了小腸液的主要 成分。(一)小腸液的性質(zhì)、成份和作用小腸液是由小腸粘膜中的小腸腺所分泌。呈弱堿性,pH值約為8-9 。成人每日分泌量約1~3L。小腸液邊分泌邊吸收,這種液體的交 流為小腸內(nèi)營養(yǎng)物質(zhì)的吸收提供了媒介。小腸液中除水和電解質(zhì)外,還含有粘液、免疫蛋白和兩種酶:腸 激酶(能激活胰蛋白酶原,見前文)和小腸淀粉酶。過去認(rèn)為小 腸液中還含有其它各種消化酶,但現(xiàn)已證明,其它各種消化酶并 非小腸腺的分泌物,而是存在于小腸粘膜上皮細(xì)胞內(nèi)。它們是分 解多肽為氨基酸的幾種肽酶以及分解雙糖為單糖的幾種二糖酶。
